Oxygen and PSA

How a hospital makes its own oxygen, and what to check before it does.

13 answers

What is a PSA medical oxygen generator?

A PSA (pressure swing adsorption) oxygen generator makes medical oxygen on site from ambient air, which is about 21% oxygen and 78% nitrogen.

Compressed, filtered and dried air flows into a column of zeolite molecular sieve that holds back nitrogen under pressure, so oxygen-rich gas passes on to a buffer tank. Two columns take turns: while one produces oxygen, the other depressurises and releases its nitrogen, so the supply does not stop. Ultra Controlo’s PSA plants are called ULTRAOX; each is built around ULTRACON oxygen concentrators with two molecular sieve columns.

What purity is PSA oxygen, and is it medical grade?

Single-stage PSA plants such as ULTRAOX produce Oxygen 93, a medical oxygen with its own monographs in the European Pharmacopoeia (Oxygen (93 per cent), 2455) and the US Pharmacopeia (Oxygen 93 Percent).

It contains 90.0% to 96.0% oxygen by volume, the rest mostly argon and nitrogen. ULTRAOX SC and ST plants deliver 94% (±1%) oxygen to European Pharmacopoeia quality; their datasheets give residual contents at initial operation of CO2 below 300 ppm, CO below 5 ppm and a working dew point of −46 °C. Oxygen made by cryogenic distillation, delivered as liquid or in cylinders, follows a different monograph, Oxygen (0417), at 99.5% or more.

Is PSA oxygen approved for hospital use?

Oxygen 93, the grade single-stage PSA plants produce, has its own monographs in the European and US pharmacopoeias, and the World Health Organization published technical specifications for PSA oxygen plants in June 2020. Where it may be used depends on national rules, so hospitals should confirm them with their health authority.

ULTRAOX plants are CE-marked Class IIb medical devices (notified body 1639). ULTRAOX SC measures purity continuously with one or two zirconium oxide analysers, and if production leaves the limits set by the hospital, the network’s automatic changeover moves the supply to the secondary source.

PSA, cylinders or liquid oxygen: what is the difference?

The difference is where the oxygen is made and how it reaches the patient: a PSA plant makes it on site from air, while cylinders and liquid oxygen are made elsewhere and delivered by truck.

How much does a PSA oxygen plant cost, and what can it save?

Ultra Controlo sizes and quotes each ULTRAOX plant for the hospital’s oxygen demand and site.

Ultra Controlo states that on-site generation with ULTRAOX can cut oxygen costs by up to 70% over 10 years compared with cylinder or cryogenic supply, because the hospital stops paying for deliveries, cylinder rental and the oxygen left in returned cylinders. Electricity is a major running cost, and pressure drives it: each extra bar of compressor pressure adds about 7% to electricity use, which is why ULTRAOX takes in air at about 7 bar and delivers oxygen at 6 bar.

Source: ULTRAOX page; ULTRAOX datasheet 2022, p. 5.Request a quote

How long does a PSA oxygen plant last?

ULTRAOX is designed for a minimum life cycle of 150,000 hours, about 17 years of continuous operation.

Its PSA cycles are widely spaced, which means fewer valve operations and less wear, and in good conditions the zeolite can last 10 years or more before it needs replacing. Service intervals depend on the compressors: ULTRAOX SC, with oil-less ULTRACECO compressors, needs one service every 10,000 hours.

Source: ULTRAOX datasheet 2022, p. 4; ULTRAOX SC datasheet.ULTRAOX

What capacities do ULTRAOX plants cover?

Each ULTRACON oxygen generator in an ULTRAOX plant produces from 0.32 to 164 m³/h of oxygen (5.3 to 2,734 L/min) at 93% purity, depending on the model, delivered at 6 bar.

Plants are modular: one or more generators and compressors are combined to match the hospital’s demand, and capacity can be added later without replacing what is already installed. To size a plant, Ultra Controlo needs the oxygen flow required in m³/h or, for cylinder filling, the size and number of cylinders to fill per day.

Source: ULTRAOX datasheet 2022, p. 15 and p. 20; ULTRAOX SC and ST datasheets; Contact page.ULTRACON

What is the difference between ULTRAOX SC, ST and CO?

They share the same PSA oxygen generators and differ in how the air is compressed and where the plant is housed.

ULTRAOX SC uses oil-less ULTRACECO compressors. ULTRAOX ST uses ULTRASCREW rotary screw compressors. ULTRAOX CO is a complete plant built inside a shipping container, for sites with no technical room or no space for civil works. ULTRAOX treats the air before the PSA with a water separator with electronic drain, micro and sub-micro filters and an activated carbon filter.

What happens if the oxygen plant stops?

The hospital stays on oxygen: an ULTRAOX plant connects to the network as one source among others, and if its production leaves the limits set by the hospital, the network’s automatic changeover moves the supply to the secondary source, such as a cylinder bank. Ultra Controlo states the switch takes under 2 seconds, with no operator action.

Inside the plant, the Siemens controller switches compressors and generators in cascade to keep pressure stable. Every alarm has a potential-free contact for remote signalling, and an optional control box passes information to the building management system (BMS).

Source: ULTRAOX page; ULTRAOX SC datasheet; ULTRAOX datasheet 2022, p. 16.ULTRAOX

What does an ULTRAOX plant monitor?

The ULTRAOX touch screen, run by a Siemens PLC, shows oxygen purity, flow and total volume produced, oxygen pressure, air quality, equipment status, the next maintenance due, a record of every contingency, and alarms.

On ULTRAOX SC, purity is measured by one or two zirconium oxide analysers. For more parameters, Ultra Controlo’s QuPHARM wall-mounted analyser, launched in May 2026, monitors up to four at once: oxygen, carbon monoxide, carbon dioxide and moisture.

Can a PSA oxygen plant fill oxygen cylinders?

Yes: an ULTRAFIL filling station connects to an ULTRAOX plant and fills medical oxygen cylinders using an ULTRACECO HP high-pressure oxygen compressor, at up to 200 bar depending on the model.

The filling rack takes 2 to 24 cylinders, as specified. The ULTRAFIL datasheet puts the cost of a refill at about one tenth of the cost of buying the gas.

What is a containerised oxygen plant, and when is it used?

A containerised oxygen plant is a complete PSA plant built inside a shipping container and delivered ready to connect; Ultra Controlo’s version is ULTRAOX CO.

It suits sites with no technical room or no space for civil works. The site provides level ground reachable by a crane, a drain for condensate, anchoring and three-phase power, and the plant can run from a diesel generator. Ultra Controlo builds medical air the same way: an ULTRAAR ME containerised medical air system left its factory for a hospital in Milan in July 2026.

Does Ultra Controlo make oxygen plants for industry?

Yes: ULTRAOX VSA is Ultra Controlo’s industrial oxygen plant, delivering 75% to 95% oxygen at 50 kPa, with an optional oxygen booster for higher pressures.

Its life cycle is calculated at 150,000 hours. It comes on a skid or in a container, can replace liquid oxygen tanks or run as the main supply, and is designed for aquaculture, wastewater treatment, hydrogen sulphide (H2S) reduction, gold leaching in mines, pulp bleaching and industrial furnaces.

What is medical air?

Medical air is compressed air supplied through the hospital pipeline, breathed by patients and used to drive medical equipment, and the European Pharmacopoeia treats it as a medicine.

The Pharmacopoeia sets limits for its oxygen, carbon dioxide, carbon monoxide, sulphur dioxide, nitrogen oxides, oil and water content. ULTRAAR SC plants are specified to the Pharmacopoeia parameters: CO2 below 500 ppm, CO below 5 ppm, SO2 below 1 ppm, NOx below 2 ppm, particles below 1 μm, oil exempt and a working dew point of −46 °C.

What is the difference between oil-free and oil-lubricated medical air compressors?

An oil-free compressor puts no oil into the air it compresses, so no compressor oil reaches the medical air network; an oil-lubricated compressor depends on separators and filters to keep its oil out.

ULTRAAR SC is totally oil-free: it uses oil-free spiral (scroll) ULTRACECO SC compressors and is a Class IIb medical device. Oil-lubricated screw, vane or piston compressors also leave oily condensate that has to be separated before it is drained. Where screw compressors are specified, ULTRAAR ME adds air treatment certified to the Pharmacopoeia parameters and an oil/water separator for the condensate.

Can medical air be used for autoclaves and other building equipment?

No: hospital standards prohibit using medical air for industrial purposes, so autoclaves, drying, valve actuation and other pneumatic equipment need a separate compressed air supply.

Ultra Controlo builds that supply as ACIH, industrial hospital compressed air systems with Gardner Denver rotary screw compressors of 15 to 720 m³/h each, at 8 to 10 or 10 to 12 bar, with air treatment and an oil/water separator for the condensate. ACIH follows the Machinery Directive and the Portuguese ACSS ET 08/2010 specification, and can be built with oil-free compressors on request.

What is the difference between a refrigeration dryer and an adsorption dryer?

A refrigeration dryer cools compressed air so its water condenses and drains away; an adsorption dryer passes the air through a desiccant and reaches much lower dew points.

In Ultra Controlo’s range, ULTRASEC is the refrigeration dryer, with an aluminium heat exchanger that accepts inlet air up to 70 °C. ULTRADRY is the adsorption dryer, with a pressure dew point of −40 °C to −70 °C, desiccant in easy-change cartridges, a coalescing filter at the inlet and a dust filter at the outlet.

What is a medical vacuum system?

A medical vacuum system is the central plant and pipeline that provide continuous suction for clinical use across the hospital.

Ultra Controlo’s ULTRAVAC plants run fully automatically with 1 to 12 Elmo Rietschle vacuum pumps of 3 to 1,600 m³/h each, at a normal operating vacuum of −0.85 bar, with two bacterial filters and a liquid separator, vacuum vessels of 24 to 6,000 litres and analogue or QuVAC touch-screen control. Pump options include oil-lubricated vane, dry-running vane, liquid ring, side channel and dry-running claw pumps. ULTRAVAC is built to ISO 7396 and HTM 02 and delivered pre-assembled and factory tested.

What does AGSS stand for?

AGSS stands for anaesthetic gas scavenging system: the system that captures waste anaesthetic gases from the patient’s breathing circuit and removes them from the operating room, protecting staff from long-term exposure.

Its disposal system is covered by EN ISO 7396-2. Ultra Controlo’s AGSS is ULTRASEG, an active system with Elmo Rietschle side channel pumps of 50 to 300 m³/h each at a suction level of −0.2 bar, in the duplex version one pump runs while the other stands by, with terminal units whose adjustable orifice suits high-flow and low-flow systems.

Source: ULTRASEG datasheet; ISO 7396-2.ULTRASEG

Is AGSS the same as medical vacuum?

No: medical vacuum provides clinical suction at a stronger vacuum, while an AGSS removes anaesthetic gases at a much lower suction level, and its disposal system has its own standard, EN ISO 7396-2.

That standard also covers avoiding cross-connections between scavenging systems and medical gas or vacuum pipelines. In Ultra Controlo’s range, ULTRAVAC works at a normal vacuum of −0.85 bar and ULTRASEG at a suction level of −0.2 bar.

What is a medical gas pipeline system (MGPS)?

A medical gas pipeline system (MGPS) is the network that carries medical gases and vacuum from the central supply to every point of use in a hospital.

It includes the supply sources, copper pipework, manifolds, area valves, pressure regulators, alarm and monitoring panels, and the terminal units on bed-head units, wall outlets and ceiling pendants. EN ISO 7396-1 sets the requirements for its design, installation, testing, commissioning and documentation, for gases such as oxygen, nitrous oxide, medical air and carbon dioxide, and for vacuum. Ultra Controlo supplies the central plants and a complete range of MGPS components.

What is a medical gas manifold with automatic changeover?

A manifold connects banks of gas cylinders to the pipeline at a constant pressure, and an automatic changeover switches from the primary bank to the reserve bank without interrupting the supply.

Ultra Controlo supplies cylinder manifolds with automatic and semi-automatic control panels, third and fourth source control panels, a fully automatic nitrous oxide manifold, emergency boxes, and maintenance supply assemblies for working on the lines.

Source: Medical gas pipeline systems page.Manifolds and connection systems

What alarms does a medical gas pipeline need?

EN ISO 7396-1 includes requirements for the monitoring and alarm systems of a medical gas pipeline, alongside its supply and distribution.

Ultra Controlo supplies master alarm panels, alarm relay panels for zones, valves and second-stage units, low pressure switches and Siemens digital pressure displays, plus the QuGas monitoring system and the QuAir alarm panel. On ULTRAOX plants, every alarm also has a potential-free contact for remote signalling.

What is a medical gas emergency cabinet?

A medical gas emergency cabinet keeps gases flowing to a critical area of the hospital if the network that feeds it fails, without anyone having to intervene.

Ultra Controlo’s ULTRAGEC connects by derivation to the secondary network upstream of the critical area and holds 2 or 4 B50 cylinders of oxygen and medical air, a 40 m³/h vacuum pump, and a supervision monitor that watches primary and secondary network pressures. ULTRAGEC MOBILE puts the same cabinet on wheels.

What does a site need before an oxygen plant is installed?

For an ULTRAOX plant, the hospital provides a clean, level area reachable by a crane, a drain for condensate, anchoring for the equipment, three-phase power and the licences to install and operate it.

The ambient air must be reasonably free of dust, at an acceptable humidity and free of chemical contamination, the area must not be classed as electrically hazardous, and no one may smoke within 10 metres of the plant. The plant can run from a diesel generator, and the existing oxygen installation can stay in place: ULTRAOX connects to it as a new source.
